Responsibilities

  • Perform primary health care for patients by utilizing assessment and diagnosis processes.
  • Obtain patient medical and historical information, prioritize data collection based on immediate conditions or needs.
  • Conduct physical exams and assess patient mental status.
  • Develop and implement treatment, follow-up, and evaluation plans for patient outcomes.
  • Collaborate with physicians and other disciplines as appropriate, providing regular and consistent feedback.
  • Use appropriate assessment techniques and diagnostic information to derive diagnoses and develop treatment plans.
  • Order appropriate ancillary tests and document findings.
  • Prescribe medications and other non-pharmacologic interventions, including controlled substances, to promote comfort and symptom management.
  • Manage and preempt common side effects of pharmacologic intervention, recommend and facilitate non-pharmacologic strategies, and consult other team experts as needed.
  • Implement interventions consistent with the established plan of care in a safe and appropriate manner.
  • Communicate effectively with patients and families about diagnosis, prognosis, care options, and treatment plans.
  • Evaluate patient care to determine response to care/interventions for optimal patient outcome.
  • Evaluate patient capabilities and limitations related to nursing and medical interventions.
  • Provide patient and family teaching and education specific to the patient's health care, ensuring proper use of medication and health/illness prevention.
  • Maintain a professional behavior in attitudes, demeanor, and interactions to achieve the organization's mission.
  • Communicate effectively with physicians, staff, and patients, being approachable and professional in working with other health professionals, patients, and their families.
  • Maintain a professional appearance, behavior, and work environment, initiating action to correct, prevent, and/or minimize risks to the patient, family, or hospital/clinic employees.
  • Participate in performance improvement and outcome management, meeting regularly with medical directors for chart reviews, identifying concerns in patient care, and participating in quality improvement initiatives.
  • Contribute to research and analysis of PI data, developing and implementing changes in protocols, standards, policies, and practices based on current research and PI data.
  • Provide appropriate care specific to the age of the patient to ensure understanding and comfort level of treatment as outlined in the "Age Specific Criteria."
  • Serve as a resource for staff within expertise areas on content for clinical patient care and outcomes, demonstrating knowledge of EBP and research as it applies to area of expertise.
  • Assist in the development of clinical practice guidelines/standards that support implementation of EBP and current research in the specialty area.
  • Serve in leadership roles that promote continued improvement of services for palliative care patients (e.g. chairs committee, develops standards, policies, etc.).
  • Provide timely initial patient and family assessments for palliative care consults, conducting, leading, and facilitating palliative care consultation meetings and decision-making using theoretical principals, skills, and competencies of advanced nursing practice and palliative care.
  • Collaborate with other team members in patient/family conferences, communicating effectively with patient and family about diagnosis, prognosis, care options, and treatment plans.
  • Assess, document, and provide for education needs for the patient and family in a collaborative manner with other team members.

Requirements

  • Graduate of Masters of Science in Nursing (MSN) or Doctorate of Nursing Practice (DNP) program.
  • Satisfactory completion of annual mandatory education requirements.
  • Current valid Registered Nurse (RN) License, valid compact multi-state license, or a temporary permit while awaiting licensure.
  • Current Nurse Practitioner (APRN.NP.NE or ARNP.IA) licensure for work state.
  • Minimum of 5 years nursing experience.
  • Minimum of 3 years previous experience in a related clinical area.

Qualifications

  • Master's degree in Nursing (MSN) or Doctorate of Nursing Practice (DNP).
  • Licensure as a Registered Nurse (RN) with a valid compact multi-state license or a temporary permit while awaiting licensure.
  • Licensure as a Nurse Practitioner (APRN.NP.NE or ARNP.IA) for the work state.
  • At least 5 years of nursing experience.
  • At least 3 years of previous experience in a related clinical area.

Skills

  • Advanced assessment and diagnostic techniques.

Pay

Salary range: $XXX - $XXXX based on experience and qualifications.

Schedule

Work Schedule: Minimum of 2 shifts per month. M-F 8a-4p as needed, Rotating weekends and Holidays (6-12 weekends per year and 1-2 holidays per year.) Rotating on-call from 4p-8a. Casual/PRN employees work on an 'as needed' basis to meet the needs of the business. Work days/hours will often vary from week to week.
