Nurse Practitioner Palliative Care

About the Role
As our Palliative Care Nurse Practitioner, you will support patient and physician satisfaction by performing as a Physician Liaison and Patient Educator to help patients achieve optimal health outcomes while delivering ethical, professional, and compassionate care. You will provide services at or above customary practice standards, adhering to the rigorous standards and principles of medical ethics set by the American Medical Association and relevant accrediting boards. This role operates within a supportive and resourced team environment.
Responsibilities
- Identify the need for Case Management services through gathering and evaluating relevant data.
- Coordinate patient care from admission, through hospitalization, to post-discharge follow-up in collaboration with the healthcare team.
- Revise care plans to achieve optimum patient outcomes.
- Support Core Measure Education Requirements and Regulatory Agency-Required Patient Education.
- Promote Patient Satisfaction Initiatives and Teaching Requirements, including education on new medications, potential medication-related side effects, pain control, and illness-related symptoms post-discharge.
- Evaluate knowledge deficits and educate patients using teach-back principles.
- Collaborate with physicians, nursing staff, administration, Case Management, Quality Improvement, Nursing Education, and Patient Satisfaction Champions.
Requirements
- Two to five years of acute hospital experience as a Nurse Practitioner with proven competence in adult patient clinical areas.
- Hospice, Palliative Care, or Skilled Nursing experience.
- Experience with teaching and/or mentoring.
- Experience in initiation and/or application/utilization of research.
- Current California Nurse Practitioner license.
- BLS certification.
- MSN required.
- Advanced preparation in pain management.
