Nurse Practitioner or Physician Assistant - Outpatient Domain Part-Time - Primary Care

Essential Responsibilities

• Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, discharge planning and providing accurate medical record documentation for patients in physician office(s) and non-acute based settings.
• Communicates with attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice.
• Makes medical diagnoses and institute appropriate therapy within the applicable scope and standards of practice.
• Orders, interprets, and evaluates laboratory, radiological, and diagnostic studies appropriate for the patient's condition to determine and implement treatment plans.
• Affirms learning needs of patient and family and provides education based on age, culture and willingness to learn.
• Educates patients and family members regarding health and illness prevention, medication and treatment instructions, and recommend community resources as needed.
• Maintains mandatory professional continuing education, actively participates in patient safety, quality improvement, and patient engagement initiatives, and maintain professional practice consistent with organizational and advanced practice mission statements and specialty-specific privileges.
• Performs other duties as assigned or required.

Qualifications

• Required: Master's degree MSN from the practicing State Board of Nursing Approved Master's Program
• National Board Certification for Nurse Practitioners through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), National Certification Corporation (NCC), or Pediatric Nursing Certification Board (PNCB)
• Practicing-state licensure as a Nurse Practitioner Prescriptive Authority
• Within 60 days of hire: Drug Enforcement Agency Licensure, CPR – American Heart Association, Act 34 Criminal Background Clearance Certificate, Act 33 Child Abuse Clearance Certificate, Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
• Preferred: Master’s degree in Physician Assistant Studies, Current PA Physician Assistant License PA-C or board eligible. Ongoing successful recertification (NCCPA) as a PA-C
• Within 90 days of hire: Drug Enforcement Agency Licensure, CPR – American Heart Association, Act 33 Child Abuse Clearance Certificate, Act 34 Criminal Background Clearance Certificate, Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
• Four to ten years related experience and/or training; or equivalent combination of education and experience.
