Nurse Practitioner or Physician Assistant - Outpatient Domain - Palliative Care

About the Role

This position is an integral part of the care delivery system, providing specialty and critical care healthcare services in physician offices and non-acute settings. The role focuses on chronic health conditions in specific populations, adhering to applicable scope and standards of practice, and aligning with the organization’s policies, values, and mission within the CRNP scope of practice. Responsibilities may include specialty certification, advanced procedures with appropriate privileging, weekend and on-call coverage on a rotational basis, as well as e-consults and telehealth services.

Responsibilities

• Provides patient care, including recording medical history, physical assessments, developing therapeutic care plans, ordering diagnostic tests, interpreting results, discharge planning, and accurate medical record documentation (30%).
• Communicates with attending physicians regarding care plans and involves them in clinical decisions outside the CRNP’s knowledge base or scope of practice. Makes medical diagnoses and institutes therapy within applicable scope and standards (15%).
• Orders, interprets, and evaluates laboratory, radiological, and diagnostic studies to determine and implement treatment plans (10%).
• Assesses patient and family learning needs, providing education on health, illness prevention, medication, treatment instructions, and community resources (15%).
• Ensures accurate and timely documentation in the EMR to support medical coding, billing, and reporting pertinent information to the attending physician (10%).
• Maintains mandatory continuing education, participates in patient safety, quality improvement, and engagement initiatives, and upholds professional practice consistent with organizational and advanced practice mission statements (10%).
• Performs other assigned duties as required (10%).

Requirements

• Master’s degree (MSN) from a State Board of Nursing-approved program.
• National Board Certification for Nurse Practitioners through an accredited association.
• State licensure as a Nurse Practitioner in the practicing state.
• Prescriptive Authority from the practicing state (within 60 days of hire).
• Drug Enforcement Agency (DEA) Licensure (within 60 days of hire).
• CPR certification (American Heart Association).
• Act 34 Criminal Background Clearance Certificate.
• Act 33 Child Abuse Clearance Certificate.
• Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
