Nurse Practitioner or Physician Assistant, HouseCalls - Utah Statewide Traveler

This position involves conducting in-home health assessments for Medicare Advantage members, focusing on preventive care to improve health outcomes and facilitate care navigation. It offers flexible scheduling, no on-call or weekend/holiday work, and requires travel across Utah, with all expenses covered.

Responsibilities

• Perform comprehensive assessments, including medical history, vital signs, physical exams, screenings, and point-of-care testing
• Identify diagnoses, document findings, and communicate with primary care providers
• Recognize urgent situations and intervene appropriately
• Educate members on health management, medications, and social determinants of health
• Provide referrals and address social needs as necessary

Requirements

• Active licensure as a Nurse Practitioner (ANCC/AANP) or Physician Assistant (NCCPA, state license)
• Unrestricted driver's license and reliable transportation
• Ability to navigate home environments and perform physical assessments

Skills

• Preferred: 1+ years clinical experience
• Proficiency with electronic records
• Strong communication skills
• Ability to handle equipment during home visits

Pay

Salary ranges from approximately $109,500 to $164,000, supplemented by incentives and bonuses.
