About the Role
As a Nurse Practitioner or Physician Assistant in Physician Services, you'll provide primary and consultative care to geriatric and medically complex patients, often in their homes or care settings. You'll conduct comprehensive medical evaluations, develop and manage individualized treatment plans, prescribe and monitor medications, and coordinate closely with physicians, caregivers, and interdisciplinary team members. This role allows you to focus on delivering high-quality, patient-centered care that improves comfort, function, and quality of life.
Location
- Office Location: Grand Rapids, MI
- Main Service Area: Grand Rapids and surrounding areas
This role is a high-travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes.
Schedule
Monday to Friday, 8 a.m. to 5 p.m.
Requirements
- Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant
- Current State License as a Nurse Practitioner or Physician Assistant
- Reliable transportation, valid driver's license, and valid automobile insurance coverage
Preferred Qualifications
- Minimum 2 years clinical experience in a post-acute setting as a Nurse Practitioner or Physician Assistant
- Adult, Family, or Gerontology certification
- Experience with geriatric patients
Skills
- Nursing skills as defined by accepted nursing standards
- Ability to assess and respond to the needs of patients, families, and caregivers in post-acute care settings
- Skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles
- Ability to handle multiple priorities, documentation requirements, and deadlines
Pay
This role provides a base annual salary of $110,000—$125,000 USD with monthly bonus potential.
