About the role

We are seeking a passionate Nurse Practitioner or Physician Assistant to join our team in Londonderry, NH on either a full-time or part-time basis. This role offers the opportunity to provide top-tier medical care while enjoying work-life balance and the vibrancy of our diverse community.

Responsibilities

  • Obtain and review patient medical and psychosocial history relevant to care.
  • Conduct physical examinations following company guidelines.
  • Order and review laboratory studies, diagnostic procedures, and ECGs.
  • Provide preventive medicine counseling, program counseling, and education on products and supplements.
  • Adhere to prescription guidelines as outlined in the collaborative agreement with the supervising physician.
  • Maintain required documentation for patient care and medication management in the EMR according to company standards.
  • Review patient progress and adjust care plans as needed.
  • Facilitate referrals to other healthcare professionals and medical facilities.
  • Conduct follow-up calls to discuss patient progress and address concerns.
  • Weekly review of Weight Tracker Report, Slipping Report, and assigned KPIs to provide clinical oversight and mentor staff.
  • Serve as the company liaison for medical and clinical affairs at the location.
  • Participate in ongoing training initiatives and act as an in-house trainer for medical staff.
  • Identify opportunities to improve clinical operations’ efficiency, effectiveness, and capacity.
  • Develop strategic recommendations to enhance patient experience.
  • Participate in the launch of test pilots for new programs, services, and products.
  • Demonstrate project management skills by developing and monitoring project work plans.

Requirements

  • DEA Certification (Required)
  • Certified Nurse Practitioner or Physician Assistant License (Required)
