Responsibilities

• Perform comprehensive medical history reviews and physical examinations.
• Conduct health screenings and manage acute and chronic medical conditions.
• Document patient encounters accurately and thoroughly.
• Engage in advanced care planning and provide patient education.
• Collaborate with an interdisciplinary team to coordinate care.

Requirements

• Active licensure as a Nurse Practitioner or Physician Assistant.
• Current certification and DEA prescriptive authority.
• Reliable transportation and ability to navigate varied home environments.

Qualifications

Preferred candidates have clinical experience in geriatric, home health, or long-term care, with a strong ability to manage complex patient needs. Proficiency with electronic medical records is highly desirable.
