Nurse Practitioner or Physician Assistant
About the Role
Horizon Health in Martin, SD is seeking a full-time Nurse Practitioner or Physician Assistant to provide direct patient care for all patients. We are looking for candidates who are passionate about delivering high-quality, patient-centered care in a collaborative, team-oriented work environment focused on meeting the needs of individual patients while supporting and contributing to the health and wellness of our communities. This position is eligible for National Health Service Core Loan Repayment. Successful candidates must be licensed or be able to successfully obtain licensure in the State of South Dakota. Certification from an accredited organization is needed or must be obtained prior to employment.
Responsibilities
- Provides primary health care to patients, according to established evidence-based guidelines of clinical practice.
- Leads care teams to provide high-quality healthcare in accordance with Horizon Quality Improvement Plan and meet established goals and benchmarks.
- Manages long-term care for acute and chronically ill patients, including care coordination and development of care plans and self-management plans.
Requirements
- Valid South Dakota License (or ability to obtain one).
- Certification from an accredited organization (or must be obtained prior to employment).
