About the role
Humana's Primary Care Organization is seeking a full-time Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at our clinic in Jacksonville, FL. As a Nurse Practitioner or Physician Assistant at Conviva Senior Primary Care, you will be part of a collaborative, diverse team including physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. You will report to the Associate Medical Director.

Responsibilities
- Provide comprehensive care to seniors following clinical standards.
- Collaborate daily with your care team to ensure coordinated, high-quality care.
- Manage referrals, hospital/SNF coordination, DME, and home health services.
- Document care with support from quality-based coders.
- Participate in on-call rotation and support growth plans at your center.
- Apply experience with providing value-based care.
- Work on-site.
Requirements
- Master's degree in nursing with board certification.
- Active, unrestricted NP or PA license in the state of Florida.
- Experience in value-based care, primary care, or Medicare Advantage settings.
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain.
- Familiarity with HEDIS quality indicators and Medicare guidelines.
Pay
$123,800 - $170,300 per year. This job is eligible for a bonus incentive plan based upon company and/or individual performance.
