About the role

Our Nurse Practitioner/Physician Assistant (APP) role focuses on functional recovery and rehabilitation care for patients in skilled nursing facilities, with inpatient rehabilitation opportunities available if desired.

Responsibilities

• Support functional recovery and rehab care for patients in skilled nursing facilities.
• Provide inpatient rehabilitation services when desired.
• Work alongside supervising physicians to develop rehab and recovery plans.
• Utilize efficient tech tools and support staff to maximize patient‑focused time.

Requirements

• Board Certified or Board Eligible (Nurse Practitioner or Physician Assistant).
• Current licensure in the state of Michigan.
• New graduates are welcome.
• Willingness to practice at multiple locations nationwide (specify preferred location).
• Physical demands: prolonged standing, walking, sitting; ability to lift up to 50 lb; use of required PPE; compliance with facility vaccination requirements; potential exposure to bloodborne pathogens, needle pricks, and chemical hazards.

Pay

Base salary range: $125,000 – $175,000, plus productivity‑based bonus (more volume yields more income, set at your own pace).
