This position provides medical care and treatment services with an emphasis on chronic disease management and promotion of health and palliative care for patients not best served in a clinic setting. Care is provided in patient residencies, as well as through audio and video sessions. This position may focus on regular care, providing ongoing care for patients within an assigned caseload and/or territory, including long-term strategies focused on disease prevention.

This is a 0.8 NP position (4 days a week) covering central, south, and west Salem areas.

Responsibilities

  • Provide diagnosis, treatment, consultation, and follow-up care for patients, including critical, prolonged, and long-term healthcare issues.
  • Perform comprehensive health assessments, including physical and psychosocial histories; identify actual and potential health needs based on medical, mental/emotional, financial, and social factors.
  • Make referrals to appropriate social or medical agencies and practitioners.
  • Identify, evaluate, and address disease prevention, acute changes in condition, maintenance of chronic diseases, and appropriate end-of-life care per patient and family goals.
  • Develop and implement proactive, patient-centered care plans with assistance from the care team.
  • Order diagnostics, prescribe medications with follow-up and reassessment, and perform surgical procedures as appropriate under certification.
  • Approve medication refills and home health orders; respond to caregiver requests to ensure continuity of care.
  • Provide compassionate anticipatory guidance through decline due to chronic disease and high-quality palliative care focused on patient and family goals.
  • Educate patients and families on goal-based preventative, chronic, or urgent care needs, as well as prescribed medications and treatments.
  • Develop and maintain working relationships with lay and professional caregivers, including care facilities, to ensure continuity of care and appropriate implementation of orders.
  • Deliver quality care while maintaining all company productivity metrics.
  • Practice within the scope of license as required by law and regulation.
  • Participate in the after-hours call schedule.
  • Communicate effectively with medical staff on consultation, collaboration, and referrals.
  • Determine the need for specialty and rehabilitative services and make appropriate referrals.
  • Oversee care plan and case management activities for assigned patients.
  • Attend and participate in required staff meetings and ad hoc committees.
  • Provide coverage for clinical colleagues in their absence.
  • Participate in quality improvement activities as assigned.
  • Complete and maintain medical records according to agency protocols and quality metrics.

Requirements

  • Master’s degree from an accredited advanced nurse practitioner program.
  • Licensed nurse practitioner in the state of Oregon (ANCC or AANP certified).
  • Drug Enforcement Administration (DEA) license.
  • Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle.
  • CPR certification at hire or within 6 months of position start.

Preferred Qualifications

  • Minimum 2 years of related clinical experience.

  • Walk, crouch, crawl, and reach for at least 3 hours/day.
  • Operate a motorized vehicle in all kinds of weather conditions.

Working Conditions

  • Work environments include indoor/office, community, and outdoor settings.
  • Member/patient-facing interactions occur telephonically and in person.
  • Hazards may include physical, ergonomic, and biological risks.
  • Use of general office equipment.
  • Travel outside of the workplace at least weekly; personal vehicle may be used (driving infractions monitored per organizational policy).
  • Work location: Community 4 days/week, member-facing.
  • Employees assigned to Marion or Polk counties must live within 15 miles of their county assignment.
  • Flexible schedule, including evenings and weekends.

Pay

Estimated hiring range: $140,940.00 – $172,260.00 (pro-rated for 0.8 FTE position). Bonus target available.
