About the Role

Emplify Health by Gundersen is seeking an Advance Practice Clinician to join its outpatient palliative medicine practice. The clinician will serve patients in La Crosse and outreach sites throughout the region, working in close collaboration with other specialties.

• Three closely linked services: an upstream outpatient palliative care clinic, a 24/7 inpatient service, and a hospital‑owned hospice.
• Provides symptom‑control expertise, support for goals‑of‑care discussions, and advance care planning for patients with serious illness.
• Outpatient clinic operates Monday – Friday, 8 am – 5 pm.
• Supported by a robust multidisciplinary team including spiritual care, social services, care coordinators, nursing staff, and more.

Requirements

• Completed an accredited advanced practice program.
• Eligibility to obtain a license to practice in the multi‑state region with no restrictions or limitations.
• Work under the direct supervision of a physician.
• May be required to provide outreach services as part of the role.
• Teaching students, residents, fellows, or clinical staff is an expectation.

Schedule

Outpatient clinic operates Monday through Friday, 8 am to 5 pm.
