About the role
Medical Director Services PC is seeking an experienced Nurse Practitioner / Physician Assistant for a skilled nursing facility in Schenectady, NY. New graduates are welcome; extensive training is offered.
Responsibilities
- Assure delivery of quality care services to all patients in a respectful and professional manner
- Contribute to physician effectiveness by identifying short- and long-range patient care issues; provide information and commentary pertinent to deliberations; recommend options and courses of action; implement physician directives
- Assess patient health by interviewing patients; performing physical examinations; obtaining, updating, and studying medical histories
- Document patient care services by charting in patient and department records
- Perform therapeutic procedures by administering injections and immunizations; suturing; managing wounds and infections
- Instruct and counsel patients by describing therapeutic regimens; giving normal growth and development information; providing counseling on emotional problems of daily living; promoting wellness and health maintenance
- Provide continuity of care by developing and implementing patient management plans
- Maintain a safe and clean working environment by complying with procedures, rules, and regulations
- Protect patients and employees by adhering to infection-control policies and protocols
- Comply with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions
- Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies
- Develop health care team staff by providing information, educational opportunities, and experiential growth opportunities
- Contribute to team effort by accomplishing related results as needed
- Perform other duties as assigned

About Us
Medical Director Services was founded in 2016 to emphasize value-based care while treating residents with higher acuity. We prioritize quality and customer service, focusing on treating in place when possible and reducing unnecessary hospitalizations. Our team is experienced in providing medical services in skilled nursing facilities, understands effective SNF care, and is familiar with the survey process. We currently serve facilities with specialty units and provide appropriately experienced medical personnel to treat residents on ventilators. Providers routinely coordinate with surrounding hospitals to ensure continuity of care. We emphasize reducing the number of medications and requirements with narcotics and psych meds, and ensure thorough documentation when certain medications are deemed appropriate to avoid facility citations.
