Nurse Practitioner, NP / Physician Assistant, PA - Advanced Heart Failure
About the role
The Advanced Heart Failure APP is an advanced practice clinician who performs acts of evaluation, diagnosis, and treatment in collaboration with attending physicians to manage the health outcomes of patients with Advanced Heart Failure. This role involves independent judgment and decision-making as well as consultation and collaboration with other members of the health care team. The Advanced Heart Failure APP enhances the continuity of quality patient care; amplifies communication between the physician, family members, and the health care team; and extends the capabilities of the medical care team. This position is critical in leading quality and evidence-based care initiatives that will result in achieving optimal patient outcomes and improved patient satisfaction for the Advanced Heart Failure patient population.
Responsibilities
- Perform evaluation, diagnosis, and treatment of patients with Advanced Heart Failure in collaboration with attending physicians
- Exercise independent judgment and decision-making in patient care
- Consult and collaborate with other healthcare team members
- Lead quality and evidence-based care initiatives to improve patient outcomes and satisfaction
Requirements
- Two (2) years of Cardiology experience as a Nurse Practitioner or Physician Assistant
- Advanced Heart Failure experience highly preferred
- VAD (Ventricular Assist Device) Coordinator experience highly preferred
- Completion of a Nurse Practitioner or Physician Assistant program
- MSN required
- Licensed by the Pennsylvania State Board of Nurse Examiners as a Practitioner, or by the Pennsylvania State Board of Medical Education and Licensure
- Certification by the NCCPA (National Commission for Certification of Physician Assistants) or (CRNP) Certified Registered Nurse
- BLS certification required
- May require ACLS certification
- May require PALS (pediatric advanced life support)
- Successful completion of the MLH credentialing process through the Medical Staff office, with temporary or permanent privileges approved
- Position requires wearing a respirator as part of the job requirement

Schedule
- Full Time, 40 hours/week
- Monday - Friday, 8-hour day shifts
- No weekends or holidays
