Position Summary We're hiring a licensed Nurse Practitioner or Physician Assistant to be at the forefront of providing primary and preventative care to our patients. Working in collaboration with our physician, you'll perform patient evaluations, diagnose and treat illnesses, manage chronic conditions, order and interpret labs, and prescribe treatment — delivering excellent, personalized care to individuals and families. Job Responsibilities Work in collaboration with our physician to assess the physical and health needs of patientsProvide primary, acute, and preventative care across the lifespan through assessment, diagnosis, and treatment of illnesses and injuriesOrder, interpret, and record results of clinical and lab tests, and act on resultsPrescribe medications and recommend other therapeutic forms of treatmentDiagnose and manage chronic disease; provide ongoing patient management and ensure use of best practices and evidence-based careAdvise patients on health and wellnessCollaborate with and support nursing staff and medical assistants; help drive practice improvements across the organizationObserve patients' progress and record observations in the patient's medical recordPromote patients' independence by teaching patients and families to understand their conditions, medications, and self-care skillsEngage in continuous education to stay ahead of developments in the field Qualifications Active Utah APRN/NP license or PA license in good standing (or eligible to obtain)NP: Master's degree (MSN), building on RN education and licensure, with national board certification (e.g., FNP); PA: master's-level PA program with current NCCPA certificationDEA registration (or ability to obtain)Experience in primary care or family practice preferredStrong communicator who thrives in a collaborative, team-based settingComfortable working in an electronic health record (EHR) Location Prime Meridian Health 360 S 1300 W, Pleasant Grove, UT 84062 (385) 440-1400 Prime Meridian Healthcare is committed to employing a diverse workforce.
