Job Summary
As a Nurse Practitioner, you’ll use your clinical expertise to assess, manage, and coordinate the healthcare of chronically ill geriatric participants. You’ll work closely with physicians, medical leadership, and clinical operations to ensure safe, accurate, and high-quality care while supporting an interdisciplinary team.
Duties/Responsibilities
- Practice within defined scope and regulatory guidelines, including prescriptive and practice authority.
- Perform health histories, physical exams, patient education, and follow-up on diagnostic results.
- Identify urgent or emergent medical conditions and initiate appropriate interventions.
- Collaborate with physicians and interdisciplinary team members to support optimal participant outcomes.
- Support clinical operations, including documentation, scheduling, and charting.
- Participate in quality improvement initiatives and care planning activities.
- Serve as a clinical resource for staff, participants, and families.
- Contribute to 24-hour care coordination.
- Attend meetings and complete other duties as assigned.

Requirements
- Must be licensed as a Certified Nurse Practitioner in the State of practice.
- One year of experience working with the frail and elderly population or willing to participate in extensive training as needed.
- Requires valid State driver’s license with a clean driving record.
Physical Requirements
- Must be able to move intermittently throughout the workday.
- Complete an in-depth orientation to the PACE model of care that may require overnight travel.
- Subject to lifting, carrying, and supporting participants.
- Must be able to drive a motor vehicle.

Education and Experience
- Must have a Bachelor’s degree in Nursing or related field.
- Must have completed a nurse practitioner program accredited by the Commission on Collegiate Nursing Education (CCNE) or the Accreditation Commission for Education in Nursing (ACEN).
- Must be licensed as a Certified Nurse Practitioner in the State of practice.
- One year of experience working with the frail and elderly population or willing to participate in extensive training as needed.
