Nurse Practitioner, HouseCalls - Missouri Statewide Traveler

Responsibilities

• Conduct comprehensive in-home health assessments, including medical history, medication reconciliation, vital signs, and physical exams
• Identify, document, and communicate diagnoses to primary care providers for care management
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on disease management, medications, and adherence
• Address social determinants of health and provide referrals as needed

Qualifications

• Active, unrestricted licensure in Missouri
• Relevant clinical experience (preferred in family, geriatric, or home health)
• Certification from ANCC or AANP for Nurse Practitioners
• Valid driver’s license and reliable transportation
• Ability to navigate varied home environments and perform assessments requiring sitting, standing, or kneeling

Skills

• Proficiency with electronic medical records and technology
• Strong communication skills with geriatric or Medicare populations
• Ability to transport equipment up to 30 pounds and navigate stairs during home visits
