Role: Hospice Nurse Practitioner (PRN) Responsibilities Provide compassionate, home-based care to patients with life-limiting illnessesConduct comprehensive assessments and manage pain and symptomsDevelop and implement individualized care plans in collaboration with an interdisciplinary teamEducate and support patients' familiesEnsure regulatory compliance and accurately document care in the EHR Qualifications & Skills Current Nurse Practitioner licensure with hospice or palliative care experience preferredStrong clinical assessment and pain management skillsExcellent communication and family support abilitiesAbility to work independently with flexibilityCommitment to patient-centered, compassionate care within a mission-driven environment
