About the Role

The Nurse Practitioner in Hospice and Palliative Care in the Athens area plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses. This position focuses on managing complex symptoms, providing pain relief, and enhancing quality of life for patients and their families during end-of-life care. The Nurse Practitioner collaborates closely with interdisciplinary teams to develop and implement individualized care plans that address physical, emotional, and spiritual needs. They serve as a key healthcare provider, offering education, support, and guidance to patients and caregivers while ensuring compliance with regulatory standards. Ultimately, this role aims to promote dignity, comfort, and holistic well-being for patients in hospice and palliative settings.

Responsibilities

  • Conduct comprehensive patient assessments to evaluate physical, emotional, and psychosocial needs.
  • Develop, implement, and adjust individualized care plans in collaboration with physicians, nurses, social workers, and chaplains.
  • Manage and monitor symptom control, including pain management and other distressing symptoms.
  • Provide education and counseling to patients and families regarding disease progression, treatment options, and advance care planning.
  • Document patient care activities accurately and maintain compliance with healthcare regulations and organizational policies.
  • Participate in interdisciplinary team meetings to coordinate care and ensure continuity.
  • Serve as a patient advocate to facilitate communication between patients, families, and healthcare providers.
  • Support bereavement services and provide emotional support to families during and after the patient’s end-of-life journey.
