Nurse Practitioner – Gerontology, Physician Assistant – Geriatrics

Requirements

• Geriatric experience (majority of patient population)
• IM/FM PA or NP
• Locums position in Seaside, OR

Schedule

• Coverage Type: Outpatient
• Schedule Type: Clinic only
• Call Type: N/A
• Schedule (per week): M‑F (consider 3‑4 days a week as well)
• Dates of Need: November start
• Shift Times: 8 am‑5 pm
• Lunch: N/A
• Provider Minimum Shifts Required: 2 weeks per month minimum (ideally full‑time)
• Guaranteed Hours per Day: 8 hours
• Overtime threshold: 9 hours worked
• Rates to be Filled Out on Bid: Clinic only – Regular, Orientation, Overtime, Holiday

Credentialing

• Current CV must be uploaded with bid
• Active license required
• DEA required
• Board certification: BC or truly BE
• Required certification: BLS

Additional Details

This site has a combination of in‑clinic and community patients who are seen in their home. For off‑site visits, this will involve driving to the patient’s residence which can be Assisted living, SNF, or personal residence where usually a nurse from our team will join the provider for the visit. We serve exclusively adults over the age of 55 as a PACE program (Program of All Inclusive Care for the Elderly) which means you’ll be working with an 11‑person interdisciplinary team. Most patient management is handled through the EMR with other team members coordinating visits and care in the community.
