Nurse Practitioner - Full Time - Jaffrey Family Medicine
About the Role
Jaffrey Family Medicine, a practice of Monadnock Community Hospital, is seeking a compassionate and patient-centered Nurse Practitioner (ARNP) to join our dedicated primary care team. This is an excellent opportunity to practice independently while collaborating closely with experienced physicians and healthcare professionals who share a commitment to delivering exceptional care to our community. You will build lasting relationships with patients of all ages, promote wellness, manage chronic conditions, and help improve health outcomes in a welcoming and supportive environment.
Responsibilities
- Conducting complete patient histories and physical examinations
- Assessing, diagnosing, and managing acute and chronic medical conditions
- Developing individualized treatment plans and coordinating patient care
- Ordering and interpreting laboratory tests and diagnostic studies
- Prescribing treatments and monitoring patient progress
- Providing preventive care and patient health education
- Collaborating with physicians and other healthcare providers to ensure high-quality, coordinated care
- Making appropriate referrals to specialists and community resources
- Triaging patient calls and providing clinical consultation when appropriate
- Monitoring and managing patient care between visits
- Participating in community outreach and health education initiatives
- Maintaining accurate and timely documentation within the electronic medical record
Requirements
- Graduate of an accredited School of Nursing
- Successful completion of an approved Nurse Practitioner program
- Current New Hampshire Advanced Registered Nurse Practitioner (ARNP) license
- Current DEA registration
- Eligibility for professional liability insurance coverage through MCH
Preferred Qualifications
- One or more years of experience in a clinical or ambulatory practice setting
