Nurse Practitioner - Full-time (Home Health)

About The Role

Transform home-based care—from wherever you are. As Circadia's full-time tele visit NP, you'll conduct remote assessments for home health patients, act on predictive health signals in real time, and coach care teams through virtual touchpoints to drive earlier interventions and reduce preventable hospitalizations. Please note this role is hybrid — you'll spend three days a week seeing patients in their homes and two days a week delivering care via telehealth.

How Success Is Measured (KPIs)

• Visits: Target 25+ patient tele visits/day
• Flagged Patients are addressed within 24 hours
• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter
• Average adoption & impact: Demonstrated workflow adherence across patient panels; trend toward fewer falls/hospitalizations and improved home health star ratings over time

Must-Have Qualifications

• Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute
• Eligible for Medicare participation; company bills on your behalf
• 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently
• Comfortable conducting telehealth visits and navigating virtual care platforms
• Thrive in a dynamic, evolving startup environment

Nice-to-Haves

• Tech-savvy: Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools
• Spanish preferred

Pay

$140,000 - $160,000 a year
