Nurse Practitioner for In-home Visits

Join Signify Health, part of CVS Health®, as a board-certified nurse practitioner contractor to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Our mission is to build trusted relationships to make people healthier through one-on-one connections.

About the role

• Conduct in-home visits focusing on a person’s overall health and well-being, identifying potential barriers to care.
• Perform medication and medical history reviews, along with physical evaluations in the member’s home.
• Use a pre-populated, patient-specific form for each evaluation; no prescribing medicine, ordering lab tests, or altering treatment regimens.

Pay

• Earn $80 per completed assessment, with potential weekly earnings of $2,500 to $4,000 (average 6-9 visits daily).
• Increase per-visit rate up to 135% with our Market Multiplier.
• Additional compensation via pay-per-test model for visits where members qualify for diagnostic and preventive tests.
• Availability incentives based on weekly availability.

Schedule

• Flexible shifts available Monday through Sunday (no nights or weekends required).
• Short-term or long-term opportunities.
• Pre-scheduled in-home visits with health plan members.

Requirements

• Board-certified nurse practitioner with state license required.
• Medicaid license or willingness to enroll (with our support) strongly preferred.
• Independent contractor (1099) status.
