Nurse Practitioner (FNP) PRN Per-visit HRAs

About the role

Family Nurse Practitioner required to see pediatric and geriatric patients. Medicare and Medicaid enrollment is mandatory. Responsibilities

• Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine physical, mental, and social needs present at the time of the visit.
• Take patient history, perform examinations, and determine diagnoses.
• Provide written documentation of patient visits per NCQA standards.
• Take patient vital signs as necessary.
• Place case management referrals and communicate with the primary care physician (PCP) as needed.
• Communicate with patients, caregivers, agency nurses, other providers, and vendors to ensure proper diagnosis and care coordination.
• Perform all clinical duties while observing OSHA Universal Precautions.
• Maintain patient confidentiality.
• Attend required meetings, in-services, and participate in committees as requested.
• Participate in professional development activities and maintain professional licenses and affiliations.

In this role, you may work with teammates, physicians, medical staff, patients, caregivers, agency nurses, providers, and vendors.

Requirements

• Active/unrestricted nurse practitioner license to practice in coverage states.
• Board certification in one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA).
• Active BLS Certification.
• Current enrollment in Medicare and Medicaid.
• Must maintain a valid driver’s license and good driving record.
• Outstanding EHR (Electronic Health Record) skills.

Preferred Qualifications

• Geriatric training or experience.
• Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public.

Working Conditions

• Adaptability to differing weather conditions and patients’ home/residential environments.
• Full range of body motion, including handling and lifting patients.
• Manual and finger dexterity, eye-hand coordination, normal visual acuity, and normal hearing.
• Ability to stand, bend, walk, and climb stairs.
• Regular lifting and carrying of items weighing up to 50 pounds.
• Ability to ride in an automobile or van up to 150 miles daily in urban and/or rural settings.
• Ability to drive, if necessary.
