Position Specifics

This is a unique opportunity for an experienced APP who enjoys variety, collaboration, and making a meaningful impact on access and care coordination across primary care practices.

Position Highlights

• Provide acute patient care at UPMC Primary Care practices within a defined region, filling in during periods of provider leave, transition, or high demand.
• Deliver same-day acute care during business hours, using a patient-centered medical home (PCMH) model.
• Support care teams by managing Epic in-basket responsibilities, including lab results, refill requests, and patient communications.
• Collaborate with office staff, physicians, and other APPs to ensure continuity, safety, and timeliness of care.
