About the role
As a Nurse Practitioner in a senior-focused primary care setting, you will deliver comprehensive health promotion, diagnosis, and treatment for acute and chronic conditions. You will prescribe medications, examine patients, coordinate services, and support a team-based care model led by physicians.
Responsibilities
- Evaluate and treat patients according to care standards
- Monitor care quality through data and chart reviews
- Coordinate patient services, including referrals and home care
- Lead and participate in daily team huddles
- Complete medical documentation accurately and timely
- Ensure compliance with policies and participate in service growth opportunities
- Contribute to quality improvement and participate in on-call programs
Qualifications & Skills
- 2–5 years of relevant experience
- Board-certified nurse practitioner with active, unrestricted license
- Strong communication and interpersonal skills
- Ability to adapt to a value-based care environment
- Proficiency with EMR and basic computer skills
- Certification to prescribe medication and TB screening eligibility
Preferred Qualifications
- Active DEA, Medicare, and Medicaid provider numbers
- Experience managing Medicare Advantage panels and understanding Medicare guidelines
- Knowledge of HEDIS quality indicators
