Nurse Practitioner- Fee for Service Weekends
TruHealth, a division of American Health Partners, is seeking an advanced practice provider (Nurse Practitioner or Physician Assistant) to deliver personalized, coordinated healthcare to aging populations in nursing homes or assisted living communities. This role focuses on improving patient outcomes and preventing rehospitalization through independent clinical judgment and collaboration with supervising physicians and facility partners.
About the Role
As an advanced practice provider, you will implement Treat in Place Protocols in compliance with federal and state regulations, treat acute and chronic conditions, and work closely with health plan leadership. Responsibilities
- Assess patients’ medical and mental health needs and develop proactive care plans that address conditions and barriers to care.
- Ensure compliance with local, state, and federal clinical service regulations.
- Prescribe medications, order lab work, diagnostic procedures, and consultations.
- Monitor patient compliance and response to treatment, modifying plans as needed.
- Collaborate with a team of providers and case managers to improve outcomes for long-term care residents.
- Provide training and advice to facility staff as required.
Requirements
- Active APRN or PA license.
- Degree from an accredited APRN or PA program.
- Minimum of 3 years’ experience in clinical nursing, rehabilitation, or geriatric populations, preferably with Fee for Service experience.
- Proficiency with Electronic Health Records (EHR) and Microsoft applications (Word, Outlook, Excel).
