Nurse Practitioner- Fee for Service
Fee For Service opportunity for an advanced practice registered nurse (APRN or NP) or physician assistant (PA) who enjoys working with the aging population. Our providers work primarily within nursing homes or assisted living communities, delivering personalized, coordinated health care that improves patient outcomes and prevents re-hospitalization.
About the role
As an advance practice provider, you will exercise independent judgement to treat patients with acute and chronic conditions, consulting with a supervising physician as appropriate. You will work closely with facility partners and health plan leadership to implement Treat in Place Protocols in compliance with all federal and state regulations.

Responsibilities
- Assess patients’ medical and mental health needs and provide a plan of care that proactively manages their conditions and addresses barriers to care
- Ensure compliance with local, state, and federal agencies related to clinical services provided
- Prescribe medications and order lab work, diagnostic procedures, and consultations
- Monitor patients’ compliance and response to treatment and modify plans as needed
- May be required to provide training and advice to facility staff
Requirements
- APRN or PA license required
- Degree from an accredited APRN or PA program
- 3 years’ experience in clinical nursing or rehabilitation in geriatric populations and/or Fee for Service experience
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook, and Excel
