Nurse Practitioner- Fee for Service
About the role
TruHealth, a division of American Health Partners, employs advanced practice providers who work primarily in nursing homes and assisted living communities. As an APRN or PA, you will deliver personalized, coordinated care that improves patient outcomes and prevents rehospitalization, exercising independent judgment to treat acute and chronic conditions while consulting with a supervising physician as needed. You will collaborate with facility partners and health‑plan leadership to implement Treat‑in‑Place protocols in compliance with federal and state regulations. The organization also owns health plans for Medicare beneficiaries, psychiatric care, home health, hospice, rehabilitation, and specialty pharmacy services, offering a supportive environment for professional growth.
Responsibilities
- Assessing patients’ medical and mental health needs and providing a plan of care that proactively manages their conditions and addresses barriers to care
- Ensuring compliance with local, state, and federal agencies related to the clinical services you provide
- Prescribing medications and ordering lab work, diagnostic procedures, and consultations
- Monitoring patients’ compliance and response to treatment and modifying care plans accordingly
- Implementing Treat‑in‑Place protocols in accordance with all applicable regulations
- Providing training and advice to facility staff when required
Requirements
- Current APRN or PA license
- Degree from an accredited APRN or PA program
- Minimum three years of clinical nursing or rehabilitation experience with geriatric populations, or equivalent fee‑for‑service experience
- Proficiency with electronic health record (EHR) systems
- Working knowledge of Microsoft Word, Outlook, and Excel
Qualifications
- Preferred education: Bachelor’s degree or higher; Master’s degree or higher
- Required licensure: APRN (Advanced Practice RN) or Physician Assistant credential
