Responsibilities
- Manage a defined patient panel, ensuring quality, cost, and outcome alignment.
- Collaborate with external providers and care teams to maintain treatment and care plan consistency.
- Conduct patient history assessments, perform physical exams, order and interpret diagnostic tests, and develop personalized care plans.
- Ensure assessments and care plans adhere to best practices for chronic kidney care, including management of CKD, ESKD, and heart failure.
- Support transitions to renal replacement therapy, transplant, or conservative care, and engage in shared decision-making and end-of-life discussions.
- Accurately document diagnoses to support risk adjustment and identify care gaps related to HEDIS and preventative care.
- Present patient cases and provide clinical support during clinical rounds and interdisciplinary team meetings.
- Provide in-home, telehealth, and clinic-based patient care, including standing, sitting, walking, pushing, pulling, and lifting.
- Maintain current board certification and state-specific continuing education requirements.
- Adhere to Strive’s documentation policies and standards.
- Use advanced analytics, technology platforms, and interdisciplinary collaboration to deliver world-class care.
- Lead care transitions across various settings, focusing on reducing readmissions and ensuring continuity of care.
- Deliver comprehensive patient education on kidney disease progression, dialysis modality selection, and kidney transplant pathways.
- Develop and foster strong patient and family relationships centered around engagement, trust, honesty, empathy, and follow-through.
Requirements
- Master’s degree in Nursing or a similar field.
- 2+ years of experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or equivalent with specialization in cardiology (CHF) or nephrology (CKD).
- 2+ years of experience with Electronic Health Records (EHR).
- Current state-specific licensure.
- National Board Certification.
- Current DEA license or eligibility to obtain within 90 days of hire.
- BLS or CPR Certification.
- Efficient and reliable transportation, including an active driver's license.
- Basic computer and Microsoft Office skills.
- Internet connectivity with minimum speeds of 3.8Mbps/3.0Mbps (up/down) and latency preference.
Qualifications
- Demonstrated expertise in CKD management (Stages 3–5), ESRD care, and dialysis management (hemodialysis and/or peritoneal dialysis, HHD).
- Proven ability to lead care transitions across settings, focusing on reducing readmissions and ensuring continuity of care.
- Experience with Electronic Health Records (EHR).
- Experience with wound care certification.
- Experience using audio-visual technology platforms.
