Job Summary

The Resource Nights & Weekend (ReNW) Nurse Practitioner (NP) at Chapters Health System is an active member of the Medical Staff providing fully remote virtual serious illness care management to patients and families across the system's continuum of care.

Responsibilities

  • Provides fully remote virtual serious illness care management to patients and families throughout the system's continuum of care.
  • Consults with medical staff and other members of the interdisciplinary team to ensure optimal patient care.
  • Performs virtual assessments of patient/family/caregiver needs and manages care plans accordingly.
  • Supports staff by answering clinical questions, entering orders, and providing education to caregivers.
  • Maintains access to electronic medical records and documents interactions in a timely manner.
  • Collaborates with the primary team to ensure continuity of care and participates in discharge planning.
  • Participates in Quality Assurance/Performance Improvement activities and seeks to continually improve care practices.
  • Maintains current knowledge of research and trends in Hospice and Palliative Medicine and obtains necessary credentials.
  • Communicates and collaborates with team members, patients/families/caregivers, and partners to provide comfort and support.
  • Achieves expected productivity standards and implements time-based billing procedures for telemedicine services.

Requirements

  • Minimum of 3 years recent clinical experience, including substantial experience in hospice, palliative care, and/or acute inpatient care.
  • Masters or Doctorate degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
  • National certification, licensure, and prescriptive authority from the appropriate governing body for advanced nursing practice.
  • Basic Life Support certificate and compact RN license in the home state by date of hire.

  • Proficiency in computer telephony systems and Microsoft Office programs.
