Nurse Practitioner Clinical Liaison, Day Shift, Rehabilitation
About the role
Adventist HealthCare Rehabilitation seeks to hire a Nurse Practitioner Clinical Liaison who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing. The role involves serving as the team’s primary point and decision maker for complex patients, performing quality assurance audits, conducting pre-admission evaluations, building relationships, promoting services, and assisting with performance improvement initiatives.
Responsibilities
- Serve as the team’s primary point and decision maker for complex patients, clinical triage, and critical decision making (and communicate with referral sources, Rehab physiatry as indicated)
- Perform quality assurance audits of the Rehab Referral tracker to ensure the efficient processing of Rehab referrals
- In collaboration with the liaison(s), conduct comprehensive pre-admission evaluations, including chart reviews and face-to-face assessments, to determine eligibility for inpatient rehabilitation
- Build and maintain strong relationships with hospitals, physicians, discharge planners, and payors
- Promote facility services through educational presentations, tours, and outreach activities
- Provide updates to and facilitate obtaining feedback from referring providers as well as the Rehab Physiatry team
- Assist BD Director in resolving barriers to admission and contributing to performance improvement initiatives
Qualifications
- Active Nurse Practitioner license in Maryland required
- Minimum 5 years in acute care; prior experience on an acute care hospitalist team strongly preferred
- Strongly prefer individual with strong Medical/Surgical Acute Care experience, ICU
- Active American Heart Association Basic Life Support (BLS) certification required; ACLS preferred
- Valid driver’s license and ability to travel between hospitals
- Strong clinical judgment, communication, and organizational skills; proficiency with EMR systems; knowledge of Medicare/Medicaid and insurance guidelines
- Experience in marketing or business development within Post-Acute care settings a plus

Pay
$89,356.80 - $134,035.20
Schedule
Full-time, 40 hours a week, Days/Evenings, Monday through Friday, Shift Type: Days/Evenings, Typical Daily Schedule: 8:00 AM–4:30 PM
