Nurse Practitioner / APRN – Remote (Honolulu)

Key Responsibilities

• C Conduct comprehensive Annual Wellness Visits (AWVs) for Medicare beneficiaries via secure telehealth platform.
• F Facilitate structured Advanced Care Planning (ACP) discussions, ensuring patients' goals and preferences are clearly documented.
• R Review and assess patients’ medical histories, medications, functional and cognitive status, and preventive care needs.
• A Identify and address gaps in care, providing guidance on screenings, vaccinations, and chronic disease management.
• C Collaborate with interdisciplinary care teams and primary care providers to support coordinated, patient-centered care.
• D Maintain accurate and timely documentation in the electronic health record (EHR).
• E Ensure compliance with CMS guidelines and clinical quality standards.

Qualifications

• S Active and unrestricted Hawaii Nurse Practitioner license.
• T Minimum of 3 years of experience in clinical practice, including Annual Wellness Visits and Advanced Care Planning.
• M Strong knowledge of Medicare guidelines and preventive care protocols.
• P Comfort and proficiency with virtual care technology and EHR systems.
• C Excellent communication and interpersonal skills.
• B Bilingual in Spanish is a plus, but not required.
• I Ability to work independently and manage a flexible virtual schedule.

Preferred Skills

• G Background in geriatrics, primary care, or internal medicine.
• H Familiarity with HCC coding and risk adjustment models.
• J Experience working in a value-based care or accountable care setting.
