Nurse Practitioner (APRN) – Primary Care "House Visits"

Responsibilities

- Conduct comprehensive home visits for elderly and homebound patients - Evaluate and manage chronic and acute conditions - Perform medication reconciliation, lab reviews, and coordinate with interdisciplinary teams - Respond to urgent care needs via telehealth, as scheduled or on-demand - Complete accurate and timely documentation to support compliance and billing - Collaborate with care coordinators and specialists when needed

Requirements

- Flexible: Full-time or minimum of 2–3 fixed days/week - Primarily in-person house visits, with telehealth urgent care as needed - Competitive per-visit or daily pay rate - Active APRN-CNP license in Nevada - Must be Medicare and Nevada Medicaid credentialed (or eligible) - Minimum 1 year of experience in primary care, geriatrics, or urgent care - Reliable transportation for home visits - Tech-savvy and comfortable using telehealth platforms - Bilingual (English/Spanish) preferred but not required

Pay

The pay range for this role is: 80 - 90 USD per hour (Med-Care Providers)
