Key Responsibilities

  • Conduct comprehensive health assessments and develop individualized care plans tailored to patient needs
  • Provide wound care management including assessment, treatment planning, and follow-up
  • Order and interpret diagnostic tests, prescribe medications, and manage treatments within scope of practice
  • Educate patients and families on disease management, wound care, and prevention strategies
  • Collaborate with healthcare professionals and community resources to optimize patient outcomes
  • Document clinical care accurately and in a timely manner utilizing electronic medical records
  • Maintain compliance with clinical policies, safety protocols, and regulatory requirements
  • Travel to patient locations within designated mobile service area to provide care

Requirements

  • Current, unrestricted Nurse Practitioner (APRN) license with Family Nurse Practitioner (FNP) certification in the state of Oklahoma
  • Experience in wound care, primary care, or related fields preferred
  • Strong clinical judgment and ability to work independently in mobile and community settings
  • Excellent communication and interpersonal skills to effectively engage patients and healthcare teams
  • Valid driver's license and reliable transportation for travel within service area
  • Proficiency with electronic medical records (EMR) systems and thorough documentation skills
