Key Responsibilities
- Conduct comprehensive health assessments and develop individualized care plans tailored to patient needs
- Provide wound care management including assessment, treatment planning, and follow-up
- Order and interpret diagnostic tests, prescribe medications, and manage treatments within scope of practice
- Educate patients and families on disease management, wound care, and prevention strategies
- Collaborate with healthcare professionals and community resources to optimize patient outcomes
- Document clinical care accurately and in a timely manner utilizing electronic medical records
- Maintain compliance with clinical policies, safety protocols, and regulatory requirements
- Travel to patient locations within designated mobile service area to provide care
Requirements
- Current, unrestricted Nurse Practitioner (APRN) license with Family Nurse Practitioner (FNP) certification in the state of Oklahoma
- Experience in wound care, primary care, or related fields preferred
- Strong clinical judgment and ability to work independently in mobile and community settings
- Excellent communication and interpersonal skills to effectively engage patients and healthcare teams
- Valid driver's license and reliable transportation for travel within service area
- Proficiency with electronic medical records (EMR) systems and thorough documentation skills
