Nurse Practitioner, Advanced Practice Provider (Spanish Required)

Role: Nurse Practitioner, Advanced Practice Provider (Spanish Required) Responsibilities Provide effective, equitable primary care to adult Medicare patients, focusing on keeping them healthy and out of hospitals.Conduct office visits for routine and acute issues, administer wellness visits, and perform health risk assessments with thorough documentation.Coordinate care with physicians, specialists, testing facilities, and agencies.Lead population health initiatives and ensure preventive screenings are completed.Assist with phone triage, patient education, and outreach activities, including home visits if needed.Collaborate within a multidisciplinary team and participate in promotional activities. Qualifications & Skills Master of Science in Nursing, Doctor of Nursing Practice, or NP certification.Active, non-probationary state Nurse Practitioner license and DEA license.National certification in Family, Adult-Gerontology, or Gerontological Nurse Practice.Bilingual proficiency in Spanish.Experience in primary care, understanding of HCC documentation, ICD-10 coding, and health assessments (preferred).Strong teamwork and population health focus.
