Nurse Practitioner, Advanced Practice Provider

Responsibilities

• Provide effective, equitable primary care to Medicare adults, fostering long-term patient relationships.
• Conduct office visits for routine and urgent issues, administer wellness visits, and perform health assessments.
• Care coordination with other providers, specialists, and testing facilities.
• Lead population health initiatives, ensuring preventive screenings and health maintenance.
• Assist with phone triage, patient education, outreach, and conduct home visits as needed.
• Collaborate with a multidisciplinary team within an outpatient care setting.

Qualifications & Skills

• Master’s or Doctorate in Nursing or NP certification with active licensure and DEA registration.
• Certification in Family, Adult-Gerontology, or Gerontological Nurse Practitioner specialties.
• PREFERRED: Experience in primary care, understanding of HCC, ICD-10 coding, and health assessments.
• Strong teamwork, communication, and patient engagement skills.
• Bilingual abilities are a plus.

Additional Details

• Full-time position, approximately 40 hours weekly.
• Compensation ranges from $95,738 to $206,206 based on experience and factors.
