About the role
We are seeking a Nurse Practitioner, Advanced Practice Provider to deliver equitable, value-based primary care to adult Medicare patients in an outpatient setting.
Responsibilities
- providing exceptional primary care
- conducting routine and acute visits
- administering wellness exams
- coordinating care with specialists
- leading population health initiatives
- educating patients
- assisting with outreach
- conducting home visits as needed
Qualifications
- master's or doctoral degree in nursing
- national certification in family, adult-gerontology, or gerontological nurse practitioner specialties
- active NP license
- DEA license
- US work authorization
Skills
- primary care experience
- knowledge of HCC documentation and ICD-10 coding
- teamwork passion
- population health interest
- bilingual abilities
