About the role
Nurse Practitioner, Advanced Practice Provider – full-time role focused on patient‑centered care and community impact.
Responsibilities
- Provide effective, equitable primary care to adult Medicare patients, including routine and acute office visits, annual wellness exams, and health risk assessments.
- Collaborate with a multidisciplinary care team.
- Coordinate care with specialists.
- Participate in population health initiatives.
- Educate patients.
- Assist with phone triage and outreach.
- Conduct home visits as needed.
- Report to the Center Medical Director and work closely with operational leadership.
Qualifications
- Master's or Doctorate in Nursing.
- National certification in family, adult‑gerontology, or gerontological specialties.
- Active NP license.
- DEA license.
- US work authorization.
- Experience in primary care (preferred).
- Understanding of HCC, ICD‑10 coding, and health risk assessments (preferred).
- Bilingual skills (a plus).
