About the role
We are seeking a Nurse Practitioner, Advanced Practice Provider to deliver effective, equitable, value-based primary care to adult Medicare patients.
Responsibilities
- See patients independently, conduct office visits for routine and acute issues
- Perform wellness visits and health assessments
- Care coordination with specialists and other providers
- Build meaningful patient relationships, educate patients on their health
- Participate in care coordination, support population health initiatives
- Deliver care within a collaborative outpatient team
- Conduct home visits when needed
- Assist with phone triage and outreach
Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or NP certification
- Active state NP license
- DEA license
- US work authorization
Qualifications
- Familiarity with HCC documentation, ICD-10 coding, and health risk assessments
- Passion for teamwork and population health
- Bilingual skills are a plus

- Effective communication and interpersonal skills
- Strong clinical judgment and decision-making abilities
- Ability to manage time effectively and prioritize tasks
- Experience with electronic health records (EHRs)
