About the role
The Patient Care Manager coordinates all clinical services (planning, organizing, directing, and supervising) provided to hospice patients. As a member of the interdisciplinary team, this position oversees the day-to-day clinical operations of hospice in alignment with the organization's mission, philosophy, and goals, ensuring compliance with all applicable laws, rules, and regulations.
Responsibilities
- Monitor patient care activities to ensure competent and compassionate care is provided to patients.
- Oversee clinical operations to conform with organizational standards and regulatory requirements.
Requirements
- Licensed as a Registered Nurse in Pennsylvania.
- Baccalaureate or higher degree in nursing or a health-related field (preferred).
- Two years of clinical experience, with the last three years in a home health agency, home care setting, or health facility.
- Two to four years of experience in a patient care setting (acute care desirable) or two to four years of supervisory or administrative experience in a hospice organization, demonstrating strong knowledge of regulations.
- Strong patient assessment, patient management, and documentation skills.

Pay
