Job Summary
This position is located on the Memphis campus. The role supports care coordination activities within the ambulatory services; specifically, for the Malignant Hematology and Transplant Clinic. Performs all duties as assigned.
Responsibilities
- Provides specific clinical support to patients and caregivers through the course of care.
- Improves access to healthcare for patients and families by offering assistance to overcome healthcare system barriers through identifying and mitigating Social Determinants of Health.
- Assists with and facilitates the transition of care from outpatient to inpatient settings and from inpatient to outpatient settings.
- Develops a comprehensive, collaborative care plan, based on provider treatment plan and patient/family goals for recently discharged patients and promotes adherence to physician/provider recommendations and instructions.
- Provides individual patient/family education and self‑management support that is appropriate based on language, cognitive abilities, literacy level, learning style, cultural norms, patient preference, readiness for change and resources available.
- Identifies and implements cost‑effective problem solving, process improvement, risk assessment and the reduction of waste.
- Completes assigned goals.
Experience
- Minimum required: 3 years’ experience on current unit or in specialty area.
- Preferred/Desired: Experience with utilization review processes.
Education
- Minimum required: BSN or equivalent years' experience.
- Preferred/Desired: Master's in Nursing or related field.
Training
- Minimum required: CPR.

- Preferred/Desired: Familiar with scope of practice and licensure regulations.
Licensure
- Registered Nurse – Minimum required BLS/CPR.
