Responsibilities
- Navigates outpatient care for patients, identifying new patients as they are diagnosed and offering navigation services.
- Provides initial and periodic patient and family needs assessments, connects patients and families to available resources, and documents assessment and follow-up notes in the patients' medical record.
- Develops disease site expertise, provides patient and family education, and tracks the flow of patients through diagnostic and treatment services.
- For applicable patients, gathers all information required by the Commission on Cancer and formulates a survivorship care plan at the end of treatment, distributing and discussing it with the patient and family.
- Builds and maintains professional relationships with surgeons, other medical physicians, and professionals involved in the patient's care.
- Collaborates with physicians to identify patients for multidisciplinary care conferences and ensures the preparation, promotion, and documentation of these conferences.
- Participates in appropriate committees, ensures standards are met, and supports certification initiatives as requested.
- Represents the program's services to the community, collaborates with marketing for brochures and other materials, and provides information about services to community organizations.
Qualifications
- Graduate of a licensed school of nursing;
- RN with at least 5 years of clinical experience in a relevant clinical specialty;
- Proficient computer skills, including proficiency in MS Office;
- Strong organizational, time management, and critical thinking skills;
- Ability to work independently and exercise sound judgment in interactions with physicians, APNs, patients, and their families.
Education, Knowledge, Skills and Abilities Preferred
- Bachelor of Science in Nursing (BSN).
Licenses and Certifications Required
- NJ State Professional Registered Nurse License.
- BLS AHA Basic Health Care Life Support HCP Certification.
Licenses and Certifications Preferred
- National Certification.
Compensation
Minimum rate of $107,952.00 Annually
