About the Role
The Nurse Navigator is a nursing specialist who follows patients across the continuum of care. The Navigator coordinates, with an advanced level of clinical expertise, all aspects of treatment and care for Mercy's patients, their family and significant others, physicians, interdisciplinary team, and other support staff. This role performs duties and responsibilities in a manner consistent with Mercy's mission, values, and Mercy Signature Service standards.
Responsibilities
- Coordinate all aspects of patient treatment and care in collaboration with patients, their families, physicians, and interdisciplinary teams.
- Provide advanced clinical expertise to support patient care across the continuum.
- Engage with support staff to ensure comprehensive care delivery.
Requirements
- Active RN (Registered Nurse) license in the state of practice.
- BLS (Basic Life Support) certification required within 30 days of hire.
Qualifications
- Required Education: Graduate from a school of nursing.
- Preferred Education: Advanced Practice/master's degree in nursing.
- Required Experience: 2 years of nursing experience.
- Preferred Experience:
- Formal patient education and community educational presentations.
- Support group facilitation.
