About the Role

The Nurse Coordinator functions independently providing direct and indirect patient care based on the nursing process for the Comprehensive Digestive Disease Center. Functions as the primary contact for patients. Provides education as needed to the patients/family related to surgical care, diagnostic procedures, medication and other complex aspects of the plan of care. The nurse incorporates the psycho-motor and age-appropriate cognitive skills of the patient/family/significant other utilizing knowledge of patient assessment, planning, intervention and evaluation. Assures that the continuum of care process is complete to include: pre-procedure & surgery, therapy protocols, hospitalization, anticipated discharge needs and follow-up in the physician offices. Supports the faculty in clinic for every patient care session. Occasionally performs scheduling of office visits and procedures. The nurse coordinator is accountable for the quality of nursing practice. Provides nursing services as needed and assists with other activities as assigned to help grow the practice and the referrals to UCI.

Responsibilities

• Provide direct and indirect patient care based on the nursing process for the Comprehensive Digestive Disease Center.
• Serve as the primary contact for patients and their families.
• Educate patients and families on surgical care, diagnostic procedures, medication, and other complex aspects of the plan of care.
• Assess, plan, intervene, and evaluate patient care incorporating psycho-motor and age-appropriate cognitive skills.
• Ensure the continuum of care process is complete, including pre-procedure, surgery, therapy protocols, hospitalization, discharge needs, and follow-up.
• Support faculty in clinic during patient care sessions.
• Perform scheduling of office visits and procedures as needed.
• Provide nursing services and assist with activities to help grow the practice and referrals to UCI.

Requirements

• Must possess the skill, knowledge, and ability essential to the successful performance of assigned duties.
• Must demonstrate customer service skills appropriate to the job.
• Minimum 2 years of clinical experience.
• Excellent written and verbal communication skills in English.
• Current CA RN license.
• Basic Life Support Certification from AHA.
• Bachelor of Science in Nursing (required for external candidates).
• Ability to maintain a work pace appropriate to the workload.

Preferred Qualifications

• Bilingual in English/Spanish or English/Vietnamese (certain clinics may require this qualification).
