About the Role
Incumbent will provide nursing care to patients, coordination, and oversight in the outpatient Urology clinic. Serves as a single point of contact for patients and/or family members to effectively communicate the patient's plan of care and coordinates care with physicians and members of the healthcare team.
Responsibilities
- Provide nursing care to patients in the outpatient Urology clinic.
- Coordinate and oversee patient care, serving as a single point of contact for patients and/or family members.
- Communicate the patient's plan of care effectively and coordinate with physicians and healthcare team members.
- Design and implement patient and family education programs.
Requirements
- Minimum 1 year of experience working as an RN or 3 years of MA/LVN experience in a clinical setting.
- Experience coordinating care for patients with complicated medical diagnoses.
- Excellent written and verbal communication skills in English.
- Current California RN license.
- Current BLS certification.
- Computer familiarity using Word, Outlook, and Excel, or similar database software.
- Bachelor of Science in Nursing (required for external candidates).
- Ability to maintain a work pace appropriate to the workload.
- Ability to establish and maintain effective working relationships across the Health System.
Preferred Qualifications
- Specialty nursing certification relating to patient population.
- Previous experience working as a nurse navigator (care coordinator) for patients with complicated medical diagnoses, preferably in urology, pulmonary, and/or nephrology clinics.
- Knowledge of University and medical center organizations, policies, procedures, and forms.
