About the Role

Incumbent will provide nursing care to patients, coordination, and oversight in the outpatient Urology clinic. Serves as a single point of contact for patients and/or family members to effectively communicate the patient's plan of care and coordinates care with physicians and members of the healthcare team.

Responsibilities

• Provide nursing care to patients in the outpatient Urology clinic.
• Coordinate and oversee patient care, serving as a single point of contact for patients and/or family members.
• Communicate the patient's plan of care effectively and coordinate with physicians and healthcare team members.
• Design and implement patient and family education programs.

Requirements

• Minimum 1 year of experience working as an RN or 3 years of MA/LVN experience in a clinical setting.
• Experience coordinating care for patients with complicated medical diagnoses.
• Excellent written and verbal communication skills in English.
• Current California RN license.
• Current BLS certification.
• Computer familiarity using Word, Outlook, and Excel, or similar database software.
• Bachelor of Science in Nursing (required for external candidates).
• Ability to maintain a work pace appropriate to the workload.
• Ability to establish and maintain effective working relationships across the Health System.

Preferred Qualifications

• Specialty nursing certification relating to patient population.
• Previous experience working as a nurse navigator (care coordinator) for patients with complicated medical diagnoses, preferably in urology, pulmonary, and/or nephrology clinics.
• Knowledge of University and medical center organizations, policies, procedures, and forms.
