What You’ll Do
- Champion health and well-being by monitoring individuals, coordinating care, and ensuring communication across medical providers
- Document care thoroughly, including progress notes, daily logs, and nursing interventions
- Ensure safe medication administration following MAP standards
- Train and support program staff in essential care techniques and assistive equipment
- Promote independence through ADL support and goal-oriented skill-building
- Accompany individuals to medical appointments and community programs
- Contribute to team operations with additional responsibilities as needed

Qualifications
- Valid MA Nursing License
- Valid driver’s license and acceptable driving record
- Ability to meet physical requirements
- Strong communication & documentation skills
