About the role

We are seeking a Registered Nurse (RN) to provide patient care in home settings, collaborating with patients, families, and healthcare teams.

Responsibilities

• Conduct home visits to assess and deliver patient care
• Coordinate care plans with patients, families, and healthcare teams
• Consult with physicians and other healthcare providers
• Collect OASIS data for Home Health compliance
• Ensure medication accuracy and adherence
• Serve as the primary contact for multidisciplinary care
• Attend required meetings and training sessions
• Adhere to agency, state, and federal policies and regulations

Requirements

• Current RN license in Idaho
• Knowledge of nursing practices and patient care standards
• Current CPR certification
• Basic computer skills and ability to learn industry-specific software
• Valid driver’s license and automobile insurance

Qualifications

• Preferred: Minimum one year of clinical experience
