About the role
The Manager has overall day-to-day responsibility for directing and managing the activities of the Case Management Team. Facilitates the Hospital’s goals of reducing length of stay, improving patient care, and ensuring efficient and effective utilization of resources to provide an appropriate continuum of care for patients.
Responsibilities
- Professionally direct activities of support persons, volunteers, students, Community Senior Aides, and others.
- Participate in studies, surveys, or research initiated or approved by the Case Management Department to develop knowledge about effective treatment or improve quality of care delivery.
- Identify quality of care issues and refer to appropriate departments/services.
- Participate in conferences, workshops, and other professional development activities to maintain licensure and remain current with advances in the field.
- Participate in multidisciplinary task forces, committees, and projects.
Requirements
- BSN preferred. Degree in Nursing or an allied health field, or a suitable combination of education and experience required.
- Current NYS licensure as RN required.
- Minimum of 4 years of direct experience in utilization management, discharge planning, case management, or home care management.
Skills
- Current clinical and technical nursing skills.
- Knowledge of rules and regulations of child abuse/neglect reporting as appropriate.
- Familiarity with requirements of regulatory agencies and third-party payors.
- Demonstrated ability to use word processing, spreadsheet, and/or database programs.
- Excellent oral and written communication skills.
- Excellent interpersonal skills.
Pay
USD $136,275.43 per year.
Schedule
Full-Time, 9am – 5pm with rotating weekends.
